Step 1: Get a ballistic knife from box.

Step 2: Go to saloon and shoot dart board with ballistic knife in the center until piano starts playing.

Step 3: Go to ghost mansion through the maze to the other part of the building and you will see a lady playing piano, tip her.

Voila, you get a free perk. Not sure if you can get a 5th perk as the perk cap is generally set to 4.
